Now Offering Telehealth Visits!
Libertas Family Medicine is excited to announce a new way to care for our community: telehealth visits are now available!

We’ve made it easy and secure by using HIPAA-compliant Zoom technology, so you can connect with your provider from the comfort of home when appropriate.
While most of our other appointments can be scheduled online through our Patient Portal, telehealth visits must be scheduled directly through our nursing staff. To schedule a telehealth visit, you simply us at (320) 412-1310, or send a message through the Patient Portal

If you need to cancel or reschedule, we ask that you also call or message the clinic directly. Thank you for your understanding and for trusting us with your care.​
Not all visits are appropriate for telehealth visits.
What is appropriate for telehealth?

Depression and/or anxiety
-
​Medication refills (select medications)

​Follow up on labs for chronic disease management (diabetes, thyroid, etc)

Follow up on weight loss​

Birth control option counseling

Less appropriate for telehealth.

Any injuries or "sick" visit that would require an exam

Pain complaints

Skin concerns​
